It's easy to say there's always a way out but if you're in a position where you see no alternative then I don't find it selfish for them to want to end their own suffering. And as dappamikey said it would take some courage to go through with the act, knowing of the consequences. And I'm sure it would take some planning too, which I would think during the planning they've had time to think of the alternatives.

Suicide do to struggling health issues is completely different.
