I personally hope for a new city, where it's biker/mafia trouble, you got a guy just coming out of hospital after being unconscious for a few months??(or is it too much like SR2?)

I think it means a returning character from GTA SA and a returning character from GTA:IV. I don't know what time GTA:IV was set in, but SA was the 90s so if it is a modern day LA then the character must've aged a bit.

GTA IV was set in present time, so the character from IV would have only aged by a few years.
