I like the drug idea. Although it brings more of an RPG style into the game, it would be nice to have a drug system. We could include some drug running as a method of getting drugs too, perhaps a mission to pick them up in time or something. I've always wanted to make something like this 
Another idea could be a working bar. Ofc, it wont cost too much money to buy alcohol, but it would be nice if we could let our players get Nice & drunk! For something like this though, where probably best waiting for 0.2.5, since as I seen in one of the preview videos, it has scriptable 'drunk effects' (Probably a special action :p) We could even use the drunk animations to replace walking.
