When you die and go to heaven/hell, in what form do you go there? As an old person? As a kid? Newborn? Who sets the standards?

Well Heaven is meant to be paradise, and walking around with arthritis and no teeth (besides false teeth) and whatever else isn't paradise. And likewise being a baby forever isn't paradise (unless you're STyL4) because it'd be so boring lying around crying.
If Heaven existed and is a paradise you'd probably be in the form of a 20-30 year old.
As for Hell...well I don't think Satan would enjoy poking his pitchfork up someone as described in the first example, so maybe you'd be in a similar age range.

What about handicapped people? People born without an arm or a leg, what about them? Do they get that extra arm/leg? How does the Lord then know how the arm/leg should look like for that person?

You should know that there will be a scale for your deeds, the sins on a side and good deeds on the other.
every person will get rewarded/punished according to his/her deeds.
